Acetabular Reconstruction in Total Hip Arthroplasty of Dysplastic Hip

Abstract
Purpose: To evaluate the follow-up results of total hip arthroplasty (THA) in dysplastic hip patients with bone
defects in the superolateral wall of the acetabulum using the morselized impaction autografts or structural autografts.

Materials and methods: The study subjects comprised 31 dysplastic hip patients (34 hips), graded higher than
Crowe group II, who were followed at our center for at least 3 years (3~13.5 years). The average age was 45 years.
Morselized impaction autografts were used in 27 cases, and structural autografts in 7 Harris hip score (HSS), stability
and loosening of the acetabulum, and graft incorporation were analyzed.

Results: The average HSS was 93.6 points. The average bone union duration was 6.7 months. Revision THA was
performed in 2 structural autograft cases due to a technical difficulty. The acetabular component remained stable
without any significant radiolucent lines in the periacetabular region, linear migration, or rotation about the cup.

Conclusion: The outcome of the mid- to long-term follow-up was satisfactory in THAs using morselized
impaction autografts and structural autografts for the acetabular bone defects, except for 2 revision cases of structural
autografts.
